We are looking for an experienced Mental Health Social Worker to join Stockport's dedicated Adult Mental Health Team. This is an excellent opportunity for a qualified professional who is passionate about supporting adults with complex mental health needs, promoting recovery, and enhancing wellbeing. About the Role As part of a multidisciplinary team, you will provide statutory social work interventions to adults experiencing mental health difficulties. You will work closely with health partners and other agencies to deliver person-centred care and ensure effective risk management. Key Responsibilities: Undertake Care Act assessments, support planning, and reviews. Complete Mental Capacity Assessments and contribute to Best Interest decisions. Manage safeguarding concerns in line with local policies and procedures. Support individuals in hospital discharge and community-based recovery. Work collaboratively with mental health professionals to achieve positive outcomes. Maintain accurate and timely case recordings in line with professional standards. About You Qualified Social Worker registered with Social Work England. Substantial experience in adult mental health social work. Strong knowledge of the Mental Health Act, Care Act 2014, and Mental Capacity Act. Excellent communication and organisational skills. Able to work autonomously and as part of a multi-agency team. Working Arrangements Hybrid working: typically 1-2 days per week from home. Attendance in the office required for team meetings, supervision, and collaborative work. Initial office presence expected to become familiar with the team, systems, and local processes. Flexibility required for home visits and community work when necessary. Offer Pay rate: 30 per hour.
