Are you an experienced Care Coordinator looking for a new opportunity with an immediate start Our client, a well-established and reputable home care provider, is seeking a proactive and organised individual to join their team in Stockport. This is a key role within a fast-paced, friendly, and supportive office environment, ensuring continuity of high-quality care and smooth day-to-day operations.

The successful candidate will have previous care coordination experience within domiciliary care and be confident managing complex rotas, staff communication, and service delivery. You ll be a natural problem solver and strong negotiator capable of keeping things running efficiently while maintaining excellent relationships with carers, clients, and families. Experience using Access People Planner software would be a real advantage.

Key Responsibilities

  • Manage day-to-day coordination of care services, ensuring all visits are covered and clients receive the highest standard of care.

  • Prepare and manage staff rotas, taking into account travel time, availability, and client continuity.

  • Handle telephone and email enquiries promptly and professionally.

  • Maintain accurate client and carer records using Access People Planner.

  • Support compliance with CQC standards and assist in inspections.

  • Report KPIs and performance updates to senior management.

  • Plan ahead for holidays, sickness, and training to maintain full coverage.

  • Take part in the on-call rota, responding to out-of-hours calls and emergencies.

Person Specification

  • Essential: Proven experience in a Care Coordinator role within domiciliary care.

  • Essential: Excellent organisational, communication, and negotiation skills.

  • Essential: Confident IT user, able to learn and manage scheduling systems.

  • Desirable: Experience with Access People Planner software.

  • Strong attention to detail and ability to work effectively under pressure.

  • Flexible and proactive with a commitment to outstanding service delivery.
